Processing of special category data

 

Background

In order to provide you with treatment, you have been asked to provide SS Health & Performance with information about your health (“Your data”). This is considered ‘special category data’ which we need your consent to process under the new General Data Protection Regulation and Data Protection Act 2018.

Whilst you have the right to withhold your consent, if you choose to do so, we will not be able to provide you with the best treatment possible.

Use of your data

SS Health & Performance needs your data to:

1. Ensure it is medically safe for treatment to be provided;

2. Provide you with appropriate treatment;

3. Provide you (where appropriate) with specific remedial exercise, strength and conditioning programmes.

 

We may also need to share your data with other therapists or medical professionals, who will need your data to provide you with appropriate treatment. If this is the case, we will inform you of this need before sharing your data.

You can withdraw or change your consent at any time by contacting Steve Smith at sshealthperformance@gmail.com. Please note that all processing of your data will cease once you have withdrawn consent, other than where this is required by law, but this will not affect:

1. Any special category data that has already been processed prior to this point or

2. Any other personal data we are entitled to process under different lawful ground, which we will continue to process in accordance with our Privacy Notice.
